FAQ Question:
Where do I get an exemption certificate?

FAQ Answer:
If you suffer from certain medical conditions you may be entitled to free NHS prescriptions (see 'entitlement'). However, you do need an exemption certificate Form FP92 from your Health Authority to say that you do not have to pay.

To claim you will first need to fill in an FP92A form which can be obtained from:
  • Your General Practitioner/Medical Centre
  • A main Post Office
The certificate is valid for a period of five years, after which application for renewal must be made to the local Family Practitioners Committee.The stoma appliances met without charge, must be those which are accepted as prescribable on Prescription Form FP10.
